Poor coordination of care costs billions

Nancy Ham, chief executive officer of Medicity (an Aetna company) recently authored a guest blog for Becker’s Hospital Review. The following is an excerpt from her column.

In an ideal world, providers from different health care organizations would have the people, processes and technology to work collaboratively and efficiently to smoothly transition a patient from one care setting to another.  The reality is that many of them are not able to effectively coordinate care transitions, and that is taking a heavy toll on both healthcare economics and patient health.
